Subject: EUROPEAN CLUB COMPETITIONS –FORMULARY FOR THE INSCRIPTION OF CLUB TEAMS AND THEIR REPRESENTATIVES (PLAYERS AND OTHER)

Dear Sirs:

Following my Communication refª FG-020/2013, from the 24th. of July 2013, I’m now informing the new formulary that must be used for the inscription of

Clubs and their representatives (players and other), that should be sent by email to:

a) Fernando Graça, the CERH President - electronic address: [email protected]

b) Lluis Ferrer, the CERH 1st. Vice-President, in charge of all the European competitions - electronic address: [email protected]

c) Ricardo Oliveira, the CERH Secretary General - electronic address: [email protected]

The required formularies – that will be available, shortly, in the CERH “website” – consist of the following 3 documents:

1. FORMULARY REFª 001 –CLUB TEAM INSCRIPTION IN THE EUROPEAN COMPETITIONS (see Annex 1) This formulary should be sent to CERH until the date of 31st. of August 2013. These formularies need to be filled for each European club

competition, with all the requested information, namely: 1.1 The Club data (including the “fiscal number” and the digitalized “club logo”)

1.2 The Sorts hall and the rink characteristics

1.3 An upfront payment by bank transfer (a copy of which must be sent attached to this formulary), including the following items:

1.3.1 PARTICIPATION FEE, according to the European competition in question: Continental Cup: 1.500,00 € - Euroleague: 500,00 € - CERS Cup: 350,00 € - Women Cup: 200,00 €

1.3.2 PLAYERS FEES - issuing of a CERH licence for each player inscription (5,00 € per issued licence). Minimum inscription of 11 players: 55,00 € - Maximum inscription of 16 players: 80,00 €

1.3.3 OTHER REPRESENTATIVES FEES - issuing of a CERH licence for each other representative inscription (10,00 € per issued licence) Minimum inscription of 4 other representatives (including 1 Delegate and 1 Main coach) : 40,00 €

1.3.4 THE REFEREEING FEES: - Each “home” match: 400,00 € (= 2 x 200,00 €), that will paid upfront according to the following criteria: Continental Cup, CERS Cup and Women Cup (1 match): 400,00 € - Euroleague (3 matches): 1.200,00 €

1.3.5 OTHER FEES – All clubs with CERH pending dues must pay them before the inscription of any of their teams. According to the

CERS Statutes and Regulations, any club with pendent dues with CERH will be immediately suspended of all activity, concerning all

the competitions under the CERH jurisdiction.

Important note: The inscription formularies of all the club representatives (players and other) must be sent to CERH – after validation from the affiliated National Federation - as appendixes of the formulary for the Club Team Inscription. If that validation was not yet received by the club, a copy of both formularies will be attached to the Club Team inscription.

2. FORMULARY REFª 002 AND 003 – CLUB TEAM REPRESENTATIVES INSCRIPTION (see Annexes 2 and 3) These formularies should be sent to CERH until the 31st. of August 2013, safeguarding what is mentioned in the previous “important note”. The sending of both formularies to CERH need to be complemented by sending of the digitalized photos of all inscribed representative to:
