CONSEQÜÊNCIAS FISIOLÓGICAS
1. ↓eficácia muscular:
2. ↑esforço respiratório:
Marini JJ. Dynamic Hiperinflation. In Marini JJ, Slutsky AS. Physiologic Basis of Ventilatory Support. New York, Marcel Dekker Inc., 1998 pp 453- 90.
CONSEQÜÊNCIAS FISIOLÓGICAS
2. ↑esforço respiratório:
– Pressão negativa intra-pleural deve ultrapassar o AUTO-PEEP;
– Se o esforço é insuficiente, não ocorrerá o disparo do respirador.
Gladwin MT, Pierson DJ. Mechanical ventilation of the severe COPD. Intensive Care Med 1998, 24: 898-910.
CONSEQÜÊNCIAS FISIOLÓGICAS
3. Influência hemodinâmica
Conseqüência do AUTO-PEEP:• Aumento da pressão intra torácica;• Compressão de cava – contração abdominal• Compressão do átrio direito;• Aumento da pós carga do VD.• Hipotensão pós TOT
Dhand R. Respiratory Care 2005; 50: 110-23
CONSEQÜÊNCIAS FISIOLÓGICAS
3. Influência hemodinâmica
Ventilação espontânea/assistida;• Aumento do esforço inspiratório:
– Aumento do retorno venoso (antes de ultrapassar AUTO-PEEP);
– Desvio do septo interventricular para esquerda;– Diminuição de VS;– Aumento da pós carga de VE - ICC
Dhand R. Respiratory Care 2005; 50: 110-23
CONSEQÜÊNCIAS FISIOLÓGICAS
• Modo ventilatório;
• Fluxo;
• Volume corrente;
• Freqüência respiratória; • PEEP.

AUMENTO DO TE
MONITORIZAÇÃO:
• Pressão de pico: < 50 cm H2O (?);• Inclui a resistência;• Fluxo alto aumenta o pico, diminuindo AUTO-PEEP.
• Pressão de platô: menor que 30 cm H2O;• Exclui a resistência; complacência;• Indicador de hiperinsuflação• Ventilando em PCV
– Observar queda de VC;
• AUTO-PEEP < 15 cm H2O
Davidson AC. Thorax 2002; 57: 1079-84.

(VC ou FR):
↑ CO2
• PaCO2 < 90 mmHg;
• pH ≥ 7,20 ;
• Infusão de bicarbonato lenta;
• Contra indicações:– Gravidez;– Pressão intra-craniana elevada;
