Princípios do Switch

Click to edit Master title style

Princípios do Switch

• Switch intra classe- previsível em relação a resistência e

eventos adversos

D:A:D: ARV Exposure and Risk of CKD

• Retrospective analysis of pts with BL

eGFR > 90/mL/min (N = 23,560)

– Evaluated cumulative exposure to TDF,

ABC, ATV/RTV, LPV/RTV, other PIs and

risk of CKD

– 210 pts developed CKD

• Multivariate analysis: exposure to TDF,

ATV/RTV, and LPV/RTV significantly

associated with CKD development

– Risk greatly over 5 yrs

• Association with TDF or LPV/RTV and

CKD remains when excluding those

who stopped drugs during or before

study entry

• When TDF exposure censored, CKD

risk per yr of ATV/RTV or LPV/RTV

exposure increased substantially

DAWNING: Second-line DTG vs LPV/RTV + 2

NRTIs in Pts With Virologic Failure

▪ Interim results of an international, randomized, open-label phase IIIb study (N = 627)

– Most frequent enrolment sites: South Africa (27%), Peru, Ukraine, Brazil, Thailand, China (8% to 10% each)

investigator-selected NRTIs (2 vs < 2)

*HIV-1 RNA ≥ 400 copies/mL on 2 occasions. †After preplanned analysis (all Wk 24 and subsets of Wks 36/48 data), it was

recommended that LPV/RTV be discontinued due to differences in virologic nonresponse and PDVF favoring DTG arm. Protocol

amendment allowed pts on LPV/RTV to switch to DTG.

Wk 52

▪ Baseline characteristics (DTG vs LPV/RTV): female, 37% vs 33%; African heritage, 42% vs 36%; HIV-1 RNA > 100,000 copies/mL, 22% vs 20%

DTG para MVHIV com possibilidade de

engravidar

• MVHIV com possibilidade de engravidar não devem

receber TARV contendo DTG

eficaz (DIU, implante hormonal)

• A escolha do IP deve levar em consideração aspectos

relacionados às comorbidades, risco de toxicidade,

probabilidade de cepas resistentes e interações

medicamentosas
